2012 ACR/EULAR Classification Criteria for Polymyalgia Rheumatica (PMR)

The 2012 ACR/EULAR PMR Criteria are as follows:

• Patients scoring ≥4 without ultrasound are classified as having PMR.

• Patients scoring ≥5 with ultrasound are classified as having PMR.

• This score can be used to classify patients who present with symptoms concerning for polymyalgia rheumatica (PMR).
• It should only be used with patients who are ≥50 years of age, present with bilateral shoulder pain, and who have an abnormal ESR and/or CRP.
• You can use this score with or without ultrasound findings, but the cutoff value for classification will change.

• Be aware that the score cutoffs differ based on the inclusion or exclusion of ultrasound results. 
• This score is not diagnostic for polymyalgia rheumatica but can be used for research purposes and assistance in classification of symptoms.

• Initial therapy usually includes oral steroids but clinical judgment should be used when determining treatment pathway.
